However, when the block is performed blindly, it results in serious complications.</p> <p><bold><italic>AIM:</italic> </bold>This study aimed to evaluate the efficacy and safety of a retrobulbar block performed under ultrasound guidance compared with a retrobulbar block performed blindly during enucleation of the eyeball in children with retinoblastoma.</p> <p><bold><italic>MATERIALS AND METHODS: </italic></bold>A prospective randomized controlled trial was performed. The study included 40 patients who met the inclusion criteria. The patients were divided into two groups: 20 patients who underwent ultrasound-guided retrobulbar blockade (RBВ + ultrasound) and 20 patients who underwent blindly retrobulbar blockade (RBВ).</p> <p><bold><italic>RESULTS:</italic> </bold>There was an insignificant decrease in intraoperative opioid requirements in the RBB + ultrasound group, where the average dose of fentanyl was 4±1.4 μg/kg, and in the RBB group, 4.7±0.8 μg/kg (<italic>p</italic> &lt;0.05).</p> <p>The time before the administration of the first dose of analgesic in the postoperative period was 4.7±0.8 h in the RBB group and 11.7±3.3 h in the RBB + ultrasound group (<italic>p</italic> &lt;0.05). VAS and CHIPPS scores obtained 6 h after the end of surgery in the RBB + ultrasound and RBB groups were 1.8 (1.2; 2) and – 2.5 (3.8; 4.5) points (<italic>p</italic> &lt;0.05), respectively.</p> <p><bold><italic>CONCLUSION:</italic></bold> There was no statistically significant difference between the time of the retrobulbar blockade under ultrasound guidance and the retrobulbar regional block performed blindly. Retrobulbar blockade performed under ultrasound guidance provides a decrease in intraoperative opioid requirements, stable intraoperative hemodynamics, and longer postoperative analgesia.</p></abstract><trans-abstract xml:lang="ru"><p><bold><italic>Обоснование.</italic></bold> Ретробульбарный блок (РББ) у детей применяется при энуклеации не только для анальгезии в интра- и послеоперационный период, но и с целью профилактики окулокардиального рефлекса, послеоперационной тошноты и рвоты. Однако при проведении блокады вслепую могут возникнуть серьёзные осложнения.</p> <p><bold><italic>Цель.</italic></bold> Целью нашего исследования было оценить эффективность и безопасность РББ, выполненного под ультразвуковым наведением, по сравнению с РББ, выполненным вслепую, при энуклеации глазного яблока у детей с ретинобластомой.</p> <p><bold><italic>Материалы и методы.</italic></bold> Было выполнено проспективное рандомизированное контролируемое исследование.</p> <p>В исследование вошли 40 пациентов, которые соответствовали критериям включения. Пациенты были разделены на 2 группы: 20 пациентов, которым проводили РББ под ультразвуковым контролем (РББ+УЗИ), и 20 пациентов, которым РББ была выполнена вслепую (РББ).</p> <p><bold><italic>Результаты.</italic></bold> Выявлено незначительное снижение интраоперационных потребностей в опиоидах в группе РББ+УЗИ, где средняя доза фентанила составила 4±1,4 мкг/кг, а в группе РББ 4,7±0,8 мкг/кг (<italic>р</italic> &lt;0,05).</p> <p>Время до введения первой дозы анальгетика в послеоперационном периоде составило в группе РББ 4,7±0,8 ч, в группе РББ+УЗИ 11,7±3,3 ч (<italic>р</italic> &lt;0,05). ВАШ и CHIPPS через 6 ч после окончания оперативного вмешательства в группе РББ+УЗИ – 1,8 (1,2; 2), а в группе РББ – 2,5 (3,8; 4,5) балла (<italic>р</italic> &lt;0,05).</p> <p><bold><italic>Вывод.</italic></bold> Не выявлено статистически значимой разницы между временем проведения ретробульбарной блокады под ультразвуковым наведением и ретробульбарным региональным блоком, выполненным вслепую.
