Epidural steroid injection can be used when other conservative methods do not help. Epidural corticosteroid injection itself is a conservative method of steroid administration, with or without local anesthetics, directly into the epidural space using various approaches such as interlaminar and transforaminal. The mechanism of action of epidural glucocorticoid administration is mainly anti-inflammatory and anti-edematous. Currently in Russia, no glucocorticoid has epidural administration listed among its clinical indications.</p> <p><bold>AIM: </bold>This trial aimed to compare the efficacy of transforaminal epidural administration of particulate versus non-particulate glucocorticoids in the treatment of radicular pain in the lumbosacral region.</p> <p><bold>METHODS: </bold>The trial included 40 patients who were randomized into 2 groups using the lottery method. The first group (<italic>n = </italic>20) received transforminal epidural blocks with non-particulate steroids; the second group (<italic>n = </italic>20) received transforminal epidural blocks with particulate steroids. Pain level was assessed using a numerical rating scale, and the Oswestry Disability Index (ODI) was calculated. Normality of data distribution was tested using the Shapiro–Wilk test; differences between groups in quantitative and rank variables were assessed using the Mann–Whitney <italic>U</italic> test. Microsoft Excel and IBM SPSS Statistics were used for data analysis. Differences were considered significant at <italic>p</italic> &lt; 0.05.</p> <p><bold>RESULTS:</bold> When comparing the two groups, the pain level on the numerical rating scale in the triamcinolone group decreased over 3 months from 7.5 [7, 8] to 1 [1, 5] points, whereas in the dexamethasone group, pain decreased immediately after treatment from 7 [7, 8] to 1 [1, 1] points, but after 3 months, pain persisted at 4 [1, 6] points. This shows a trend in favor of triamcinolone in the medium term. When comparing the two groups by ODI, a significant reduction in the impact of pain on daily life was observed in both groups.</p> <p><bold>CONCLUSION:</bold> This comparative analysis of the analgesic effect of epidural particulate versus non-particulate steroids revealed a trend toward greater efficacy of epidural particulate glucocorticoids.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Боль в пояснично-крестцовой области — одна из наиболее частых причин нетрудоспособности и обращения за медицинской помощью. Эпидуральная инъекция стероидов может использоваться в качестве лечения, когда другие консервативные методы не помогают. Эпидуральная инъекция кортикостероидов по сути является консервативным методом введения стероидов с местными анестетиками или без них непосредственно в эпидуральное пространство с использованием различных подходов, таких как интерламинарный и трансфораминальный. Механизм действия эпидурального введения глюкокортикостероидов в основном противовоспалительный, противоотёчный. Ситуация в России на сегодняшний день такова, что нет ни одного глюкокортикостероида, у которого в показаниях к клиническому применению имеется эпидуральное введение.</p> <p><bold>Цель.</bold> Сравнение эффективности трансфораминального эпидурального введения партикулярных и непартикулярных глюкокортикостероидов при лечении радикулярной боли в поясничном отделе позвоночника.</p> <p><bold>Методы.</bold> Для исследования было отобрано 40 пациентов. Рандомизация пациентов на 2 группы осуществлена при помощи метода жеребьёвки. В первой группе (<italic>n=</italic>20) были проведены трансфораминальные эпидуральные блокады с непартикулярными стероидами, во второй группе (<italic>n=</italic>20) — трансфораминальные эпидуральные блокады с партикулярными. Оценка уровня боли проводилась по цифровой рейтинговой шкале и высчитывался индекс инвалидизации по опроснику Освестри (ODI). Нормальность распределения данных проверялась с использованием критерия Шапиро–Уилка, различия между группами по количественным и ранговым показателям оценивались с помощью критерия Манна–Уитни. Для анализа данных использовалась программы Microsoft Excel и IBM SPSS Statistics. Различия считали статистически значимыми при <italic>p &lt;</italic>0,05.</p> <p><bold>Результаты.</bold> При сравнении двух групп пациентов отмечается, что уровень боли по цифровой рейтинговой шкале в группе с триамцинолоном в течение 3 месяцев с момента начала наблюдения снижался с 7,5 [7; 8] до 1 [1; 5] балла, тогда как в группе пациентов с дексаметазоном отмечалось снижение уровня боли сразу после проведённого лечения с 7 [7; 8] до 1 [1; 1] балла, а через 3 месяца наблюдения боли сохранялись на уровне 4 [1; 6] баллов, что показывает тенденцию в пользу триамцинолона в среднесрочной перспективе.
