It has been hypothesized that anatomical differences in the ocular adnexa between patients with Asian and European eyelid anatomy may determine the severity of edema following retrobulbar block.</p> <p><bold>AIM</bold><bold>: </bold>This study aimed to evaluate the effectiveness of retrobulbar block and the surgeon’s comfort during surgery in patients with Asian and European eyelid anatomy.</p> <p><bold>METHODS</bold><bold>: </bold>A prospective controlled study was conducted involving 80 patients aged 51–75 years who underwent retrobulbar block for anesthesia during vitreoretinal surgery. Group 1 (n = 40) included patients with European eyelid anatomy, and group 2 (n = 40) with Asian eyelid anatomy. Changes in intraocular pressure and upper eyelid thickness were assessed at different stages of the block, along with preparation time for surgery, analgesia level, akinesia, and surgeon’s comfort.</p> <p><bold>RESULTS</bold><bold>: </bold>In group 1, the upper eyelid thickness increased by 0.9 ± 0.1 mm, whereas in group 2 it increased by 2.8 ± 0.4 mm (p = 0.000), due to postseptal infiltration of the upper eyelid tissue with local anesthetic. The mean akinesia score was 1.0 ± 0.2 in group 1 and 1.4 ± 0.6 in group 2 (p = 0.021), with higher scores indicating reduced akinesia effectiveness. In group 1, intraocular pressure increased to 17.6 ± 1.9 mm Hg after the retrobulbar block and was 15.9 ± 1.9 mm Hg after compression, which corresponded to the baseline values before the block. In group 2, maximal increase in intraocular pressure was recorded after the retrobulbar block—24.9 ± 6.3 mm Hg (p = 0.0000)—and after compression—21.5 ± 5.4 mm Hg (p = 0.0000), which exceeded the baseline values (16.2 ± 1.3 mm Hg). The presence of ocular hypertension resulted in additional preoperative preparation time aimed at reducing intraocular pressure: in 5% of patients in group 1 and 28.75% of patients in group 2. Pain assessment showed residual pain in group 2 (2.6 ± 1.3 on the numeric rating scale), 1.5 times higher than in group 1 (1.6 ± 1.5, p = 0.0001), requiring additional intraoperative analgesia. Surgeon’s comfort in group 2 was reduced due to decreased palpebral fissure width from 21.4 ± 1.1 mm to 14.8 ± 1.9 mm after the retrobulbar block, associated with anesthetic infiltration of the upper eyelid.</p> <p><bold>CONCLUSION</bold><bold>: </bold>Retrobulbar block in patients with Asian eyelid anatomy was associated with a significant increase in eyelid thickness and intraocular pressure, prolonged surgical preparation time, and the need for additional intravenous analgesia, resulting in reduced surgeon comfort during ophthalmic procedures.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование. </bold>Известно, что ретробульбарная блокада сопровождается отёком верхнего века разной степени выраженности, ассоциированным с умеренным экзофтальмом, что может оказывать неблагоприятное влияние на условия проведения хирургического вмешательства. Было предположено, что различия анатомического строения придаточного аппарата глаза у пациентов с азиатским и европейским строением век могут определять разную степень выраженности отёка при выполнении ретробульбарной блокады.</p> <p><bold>Цель </bold>—<bold> </bold>оценить эффективность ретробульбарной блокады и комфортность работы хирурга у пациентов с азиатским и европейским строением век.</p> <p><bold>Методы. </bold>Проведено проспективное контролируемое исследование с участием 80 пациентов в возрасте от 51 до 75 лет, которым выполнялась ретробульбарная блокада с целью обезболивания витреоретинального вмешательства. 1-я группа (<italic>n=</italic>40) — пациенты с европейским строением век; 2-я группа (<italic>n=</italic>40) — с азиатским строением век. Оценивали изменения внутриглазного давления, толщины верхнего века на этапах выполнения блокады, время подготовки к хирургии, уровень аналгезии, акинезии, комфортности работы хирурга.</p> <p><bold>Результаты. </bold>У пациентов 1-й группы отмечалось увеличение толщины верхнего века на 0,9±0,1 мм, во 2-й группе — на 2,8±0,4 мм (<italic>p=</italic>0,000), что обусловлено пропитыванием постсептальной клетчатки верхнего века местным анестетиком. Средний балл степени акинезии у пациентов составил 1,0±0,2 и 1,4±0,6 в группах 1 и 2 соответственно (<italic>p=</italic>0,021), где более высокие баллы говорят о снижении эффективности акинезии. У пациентов 1-й группы внутриглазное давление после ретробульбарной блокады повысилось до 17,6±1,9 мм рт. ст., а после компрессии стало 15,9±1,9 мм рт. ст., что соответствовало исходным значениям до блокады. У пациентов 2-й группы зарегистрировано максимальное увеличение внутриглазного давления после ретробульбарной блокады: 24,9±6,3 мм рт. ст. (<italic>p=</italic>0,0000), после компрессии — 21,5±5,4 мм рт. ст. (<italic>p=</italic>0,0000), что превышало исходные значения (16,2±1,3 мм рт. ст.). Наличие офтальмогипертензии привело к дополнительному времени подготовки перед хирургией с целью снижения внутриглазного давления: в 1-й группе — у 5% пациентов, во 2-й группе — у 28,75% пациентов. Анализ степени выраженности болевого синдрома выявил сохранение боли (2,6±1,3 балла по цифровой рейтинговой шкале) у пациентов 2-й группы, что в 1,5 раза превышало результаты, полученные в 1-й группе — 1,6±1,5 балла (<italic>p=</italic>0,0001), — и потребовало дополнительного обезболивания интраоперационно. У пациентов 2-й группы уровень комфортности работы хирурга был снижен за счёт уменьшения ширины глазной щели после ретробульбарной блокады с 21,4±1,1 мм до 14,8±1,9 мм на фоне пропитывания верхнего века местным анестетиком.</p> <p><bold>Заключение. </bold>Ретробульбарная блокада у пациентов с азиатским строением верхнего века сопровождалась значительным увеличением толщины века и внутриглазного давления, а также удлинением времени подготовки к хирургическому лечению и необходимостью дополнительного обезболивания внутривенно, в связи с чем отмечалось снижение уровня комфорта хирурга при выполнении оперативного вмешательства.</p></trans-abstract><kwd-group xml:lang="en"><kwd>retrobulbar block</kwd><kwd>upper eyelid anatomy</kwd><kwd>analgesia</kwd><kwd>intraocular pressure</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>ретробульбарная блокада</kwd><kwd>строение верхнего века</kwd><kwd>аналгезия</kwd><kwd>внутриглазное давление</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Shalwala A, Hwang RY, Tabing A, Sternberg P Jr, Kim SJ.
