High pain intensity in the early postoperative period often leads to increased opioid consumption and a higher risk of complications. Within the ERAS (Enhanced Recovery After Surgery) concept, increasing attention is given to safe regional analgesic techniques that reduce opioid requirements. One such method is the erector spinae plane block (ESP block), whose effectiveness in open urologic surgery is insufficiently studied.</p> <p><bold>AIM:</bold> This study aimed to evaluate the effectiveness of the ESP block as a component of multimodal anesthesia in patients undergoing open kidney surgery during the intraoperative and early postoperative periods.</p> <p><bold>METHODS:</bold> A prospective, controlled study included 116 patients divided into two groups: the main group (<italic>n</italic> = 60, with ESP block) and the control group (<italic>n</italic> = 56, without block). Patients older than 18 years with ASA physical status I–III and without contraindications to regional anesthesia were included. Primary outcomes were intraoperative fentanyl consumption, postoperative use of trimeperidine, pain intensity assessed by the numerical rating scale, incidence of nausea and vomiting, need for nonsteroidal anti-inflammatory drugs and paracetamol, and length of stay in the intensive care unit. Statistical analysis was performed using SPSS 25.0; significance level <italic>p</italic> &lt; 0.05.</p> <p><bold>RESULTS:</bold> ESP block significantly reduced fentanyl consumption: 0.027 ± 0.01 μg/(kg·min) vs. 0.046 ± 0.02 μg/(kg·min) (<italic>p</italic> &lt; 0.01) and the need for trimeperidine within the first 6 hours (5.0% vs. 55.6%; <italic>p</italic> &lt; 0.001). Pain intensity on the numerical rating scale was significantly lower in the main group at all observation points (median 0.0 vs. 3.0–7.0; <italic>p</italic> &lt; 0.001). The incidence of postoperative nausea and vomiting was 10–15 times lower in the main group (<italic>p</italic> &lt; 0.001), as was the need for nonsteroidal anti-inflammatory drugs and paracetamol. Length of stay in the intensive care unit decreased 3.7-fold (2.88 ± 0.17 h vs. 10.89 ± 3.51 h).</p> <p><bold>CONCLUSIONS:</bold> ESP block in open renal surgery demonstrates high clinical effectiveness as part of multimodal anesthesia, providing marked analgesia, reducing opioid load, and improving postoperative recovery.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование. </bold>Несмотря на достижения анестезиологии, послеоперационная боль при открытых операциях на почках остаётся значимой клинической проблемой. Интенсивность болевого синдрома в раннем послеоперационном периоде нередко приводит к повышенному потреблению опиоидов и риску осложнений. В рамках концепции ERAS (Enhanced Recovery After Surgery) внимание уделяется поиску безопасных регионарных методов аналгезии, позволяющих снизить опиоидную нагрузку. Одним из таких методов является блокада мышцы, выпрямляющей позвоночник (ESP-блокада), эффективность которой при открытых урологических вмешательствах изучена недостаточно.</p> <p><bold>Цель </bold>— оценить эффективность ESP-блокады в качестве компонента мультимодальной анестезии у пациентов, перенёсших открытые операции на почках, в интраоперационном и раннем послеоперационном периодах.</p> <p><bold>Методы.</bold> Проведено проспективное контролируемое исследование 116 пациентов, разделённых на две группы: основную (<italic>n</italic> = 60, с ESP-блокадой) и контрольную (<italic>n</italic> = 56, без блокады). Включались пациенты старше 18 лет со статусом по ASA I–III, не имевшие противопоказаний к регионарной анестезии. Основные показатели: интраоперационное потребление фентанила, послеоперационное применение тримеперидина, уровень боли по цифровой рейтинговой шкале, частота тошноты и рвоты, потребность в нестероидных противовоспалительных препаратах и парацетамоле, длительность пребывания в отделении реанимации и интенсивной терапии. Статистический анализ выполнялся в SPSS 25.0, уровень значимости <italic>p</italic> &lt; 0,05.</p> <p><bold>Результаты.</bold> ESP-блокада достоверно снижала потребление фентанила — 0,027 ± 0,01 мкг/(кг × мин) против 0,046 ± 0,02 мкг/(кг × мин); <italic>p</italic> &lt; 0,01 — и необходимость в тримеперидине в первые 6 часов (5,0% против 55,6%; <italic>p</italic> &lt; 0,001). Интенсивность боли по цифровой рейтинговой шкале была значительно ниже в основной группе на всех этапах наблюдения (медиана 0,0 против 3,0–7,0; <italic>p</italic> &lt; 0,001). Частота послеоперационной тошноты и рвоты была в 10–15 раз ниже в основной группе (<italic>p</italic> &lt; 0,001), как и потребность в нестероидных противовоспалительных препаратах и парацетамоле.
