The erector spinae plane block (ESP block) is a promising regional anesthesia technique that may reduce opioid consumption and improve recovery.</p> <p><bold>AIM:</bold> This study aimed to evaluate the analgesic efficacy of the ESP block compared with standard analgesia in patients undergoing laparoscopic sleeve gastrectomy.</p> <p><bold>METHODS:</bold> A pilot, single-center, prospective, randomized, placebo-controlled study included 20 patients who underwent laparoscopic sleeve gastrectomy: the ESP group (<italic>n</italic> = 10; 20 mL of 0.5% ropivacaine on each side at the T7 level) and the control group (<italic>n</italic> = 10; 0.9% NaCl administered using the same technique, sham block). Pain was assessed using the visual analog scale (at rest/during movement) at 2, 4, 6, 12, and 24 hours. Intraoperative hemodynamic parameters, opioid consumption, incidence of postoperative nausea and vomiting, and time to mobilization were recorded.</p> <p><bold>RESULTS:</bold> Patients in the ESP block group had significantly lower visual analog scale pain scores at all time points compared with the control group (<italic>p</italic> &lt; 0.01). Intraoperative fentanyl consumption and postoperative tramadol consumption were significantly lower in the ESP group (<italic>p</italic> &lt; 0.01). More stable hemodynamics were observed in the ESP group during the 30–120 minutes after induction, as well as a lower incidence of postoperative nausea and vomiting (10% vs. 80%). Time to first mobilization was significantly shorter in the ESP group (mean 7.1 h vs. 12.3 h, <italic>p</italic> &lt; 0.01).</p> <p><bold>CONCLUSIONS:</bold> The ESP block provides effective analgesia, reduces opioid requirements, improves hemodynamic stability, and facilitates early rehabilitation in patients after laparoscopic sleeve gastrectomy. The technique may be used as part of multimodal analgesia and ERAS protocols in bariatric surgery.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Эффективный контроль послеоперационной боли при бариатрических вмешательствах остаётся сложной клинической задачей, особенно у пациентов с морбидным ожирением. Блокада в плоскости мышц-выпрямителей позвоночника (ESP-блок) представляет собой перспективную методику регионарной анестезии, способную снижать потребление опиоидов и улучшать восстановление.</p> <p><bold>Цель.</bold> Оценить анальгезирующую эффективность ESP-блока по сравнению со стандартной аналгезией у пациентов, перенёсших лапароскопическую рукавную гастрэктомию.</p> <p><bold>Методы.</bold> Пилотное одноцентровое проспективное рандомизированное плацебо-контролируемое исследование; 20 пациентов, перенёсших лапароскопическую рукавную гастрэктомию: группа ESP (<italic>n</italic> = 10; 20 мл 0,5% ропивакаина с каждой стороны на уровне TVII) и контроль (<italic>n</italic> = 10; 0,9% NaCl по той же методике, sham-block). Боль оценивали по визуальной аналоговой шкале (в покое/при движении) через 2, 4, 6, 12 и 24 ч; фиксировали интраоперационные гемодинамические параметры, потребление опиоидов, частоту послеоперационной тошноты/рвоты и время до мобилизации.</p> <p><bold>Результаты.</bold> У пациентов группы ESP-блокады показатели боли по визуальной аналоговой шкале были достоверно ниже на всех временных точках по сравнению с контролем (<italic>p</italic> &lt; 0,01). Интраоперационное потребление фентанила и послеоперационное потребление трамадола были существенно ниже в группе ESP (<italic>p</italic> &lt; 0,01). В группе ESP отмечалась более стабильная гемодинамика в течение 30–120 минут после индукции, а также меньшая частота послеоперационной тошноты и рвтоы (10% против 80%). Время до первой мобилизации было значительно короче в группе ESP (в среднем 7,1 ч против 12,3 ч, <italic>p</italic> &lt; 0,01).</p> <p><bold>Заключение.</bold> ESP-блокада обеспечивает эффективную аналгезию, снижает опиоидную нагрузку, улучшает гемодинамическую стабильность и способствует ранней реабилитации у пациентов после лапароскопической рукавной гастрэктомии.
