Polyneuropathy, secondary immunodeficiency, thrombocytopenia grade IV refractory to platelet concentrate transfusion, Guillain-Barre syndrome, impaired the somatic status and intensified the pain syndrome. Local anesthetics epidural port was successfully implanted to improve quality of analgesia. As a result of prolonged epidural infusion of 0.2% ropivacaine with titration rate from 4 to 7 ml/hour depending on the severity of the pain syndrome, that significantly improved patient’s quality of life was achieved by reducing the intensity of pain and increasing duration of night sleep. In the early postoperative and long-term follow-up periods (14 days), there were no hemorrhagic and infectious complications associated with the use of the epidural port.</p></abstract><trans-abstract xml:lang="ru"><p>Представлен клинический случай 25-летнего больного прогрессирующей диффузной В-клеточной лимфомой с поражением вертебрального корешка S2, сопровождающимся болевым синдромом, не купирующимся системной мультимодальной анальгезией с применением опиоидов, противоэпилептических и нестероидных противовоспалительных препаратов. Развившиеся полинейропатия, вторичный иммунодефицит, рефрактерная к трансфузии тромбоцитного концентрата тромбоцитопения IV степени, а также синдром Гийена-Барре отягощали общесоматическое состояние пациента и интенсифицировали болевой синдром. С целью обеспечения должного уровня обезболивания пациенту был успешно имплантирован эпидуральный порт для введения местных анестетиков. В результате продлëнной эпидуральной инфузии 0,2% раствора ропивакаина с титруемой в зависимости от выраженности болевого синдрома скоростью от 4 до 7 мл/ч достигнуто значительное улучшение качества жизни пациента за счёт снижения интенсивности боли и увеличения продолжительности ночного сна.
