In accordance with the recommendations of the World Health Organization, short-acting morphine is the main drug used to relieve severe pain and breakthrough pain. In published studies, morphine provided adequate analgesia in the majority (&gt;90%) of the patients; however, no such studies have compared the use of morphine tablets with a control group of pediatric patients.</p> <p><bold><italic>OBJECTIVE:</italic></bold> To evaluate the effectiveness and safety of morphine tablets at a dose of 5 mg and compare them with those of morphine given intravenously at a dose of 10 mg/mL in children with severe pain syndrome in the postoperative period.</p> <p><bold><italic>MATERIALS AND METHODS:</italic></bold> A multicenter open randomized blind prospective clinical study was conducted in parallel groups of 100 patients aged 3–18<bold><italic> </italic></bold>years with severe pain syndrome in the postoperative period between September 21, 2020, and December 15, 2022. Cumulative morphine consumption, visual analog scale analgesic efficacy, patient satisfaction scores, and adverse events (AEs)<bold><italic> </italic></bold>/<bold><italic> </italic></bold>serious adverse events (SAEs) were assessed.</p> <p><bold><italic>RESULTS:</italic></bold> The comparative analysis of the average values of the total number of doses of an additional analgesic drug administered during treatment with the study drug (group 1)/comparison drug (group 2) in patients of different age groups (cohorts 1, 2, 3) did not reveal significant differences (<italic>p</italic>=0.05). This finding indicated the equal effectiveness of the used forms of drugs in both groups. Moreover, no significant differences were noted in the development of AEs associated with the study drug (group 1) compared with AEs of the reference drug (group 2) in patients of different age groups (cohorts 1, 2, 3); (<italic>p</italic>=0.05).</p> <p><bold><italic>CONCLUSION</italic></bold><bold>: </bold>The study demonstrated the effectiveness and safety of the study drug in tablets in comparison with an injection drug in children with severe pain syndrome.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> У значительного числа детей с повторяющимися эпизодами острой ноцицептивной боли развивается хроническая боль. В соответствии с рекомендациями Всемирной организации здравоохранения, морфин короткого действия — основной препарат для обезболивания при боли сильной интенсивности, а также для купирования прорывных болей. В опубликованных исследованиях морфин оказывал достаточный обезболивающий эффект у большинства (&gt;90%) пациентов, но ни в одном из них не сравнивали применение морфина в таблетированной форме с контрольной группой среди пациентов детского возраста.</p> <p><bold>Цель.</bold> Оценить эффективность и безопасность таблетированной формы препарата морфин в дозе 5 мг в сравнении с морфином в форме раствора для инъекций в дозе 10 мг/мл у детей с болевым синдромом сильной интенсивности в послеоперационном периоде.</p> <p><bold>Материалы </bold><bold>и </bold><bold>методы.</bold> Проведено многоцентровое открытое рандомизированное слепое проспективное клиническое исследование в параллельных группах у 100 пациентов в возрасте от 3 до 18 лет с болевым синдромом сильной интенсивности в послеоперационном периоде за период с 21.09.2020 по 15.12.2022 гг. Оценивали кумулятивное потребление морфина, анальгетическую эффективность по данным визуальной аналоговой шкалы, показатель удовлетворённости пациентов и нежелательные явления (НЯ) / серьёзные нежелательные явления (СНЯ).</p> <p><bold>Результаты.</bold> В результате сравнительного анализа средних значений суммарного числа доз дополнительного анальгетического препарата, введённого в период лечения исследуемым препаратом (группа 1) / препаратом сравнения (группа 2) пациентам в разных возрастных группах (когорты 1, 2, 3), значимых различий не наблюдалось (<italic>p</italic>=0,05), что свидетельствует о равной эффективности используемых форм препаратов в обеих группах.
