However, use of neuroaxial anesthesia in cardiac surgery remains controversial due to the lack of studies. Parasternal blockade is a simple, effective technique that can be applied after cardiac surgery.</p> <p><bold><italic>CLINICAL CASE DESCRIPTION:</italic></bold> This is a clinical case presenting the performance of bilateral parasternal blockade in the III–IV intercostal space using the «Single Shot» technique for a patient who underwent aortocoronary bypass surgery. Advantages of this technique are its ease of implementation, safety, and most importantly, effective distribution of local anesthetic in the parasternal interfascial space. This results in blockade of the anterior cutaneous branches of intercostal nerves from T<sub>2</sub> to T<sub>6 </sub>that innervate the surgical area.</p> <p><bold><italic>CONCLUSION:</italic></bold> Parasternal blockade is a technically simple, affordable, and promising method for analgesia after aortocoronary bypass surgery. Multicenter studies are required to further evaluate the efficacy of parasternal blockade postcardiac surgery.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование. </bold>При проведении операций аортокоронарного шунтирования (АКШ) перспективными и недостаточно изученными являются методы регионарной анестезии, которые позволяют достичь снижения интенсивности болевого синдрома в периоперационном периоде, сокращения потребности в системных опиоидах и, соответственно, способствовать улучшению результатов хирургического вмешательства. Использование нейроаксиальных методов анестезии в кардиохирургии остаётся спорным вопросом, что делает эту тему перспективной для дальнейшего изучения. В свою очередь, парастернальная блокада представляется технически простым, эффективным и перспективным методом регионарной анестезии, применимым в кардиохирургии. Исходя из вышеперечисленного, изучение эффективности парастернальной блокады при операции АКШ является многообещающим и может обеспечить разработку более эффективных методов обезболивания и улучшить результаты операции.</p> <p><bold>Описание клинического случая.</bold> Представлен клинический случай выполнения билатеральной парастернальной блокады в третьем–четвёртом межреберье по методике «Single Shot» пациенту при АКШ. Особенностью блокады является простота её выполнения, безопасность, а, главное, эффективное распространение местного анестетика в парастернальном межфасциальном пространстве, что приводит к блокаде передних кожных ветвей межрёберных нервов c Th<sub>II</sub> по Th<sub>VI</sub>, ответственных за иннервацию области оперативного вмешательства.</p> <p><bold>Заключение.</bold> Парастернальная блокада — это технически простой, доступный и перспективный метод аналгезии после АКШ.
