Material and methods. 59 children operated under caudal anesthesia for congenital dislocation of the thigh, deformities of pelvic bones, clubfoot, etc. were included in the study. The patients were divided into 2 groups depending on the type of local anesthetic used: in group I (n = 39) caudal anesthesia was performed with a 0.75% solution of ropivacaine with intravenous sedation with propofol; in the second group (n = 20), the caudal block was administered with a 0.5% solution of bupivacaine. During the operation and in the postoperative period, the indices of central hemodynamics, the concentration of cortisol and blood plasma glucose were studied, the intensity of postoperative pain syndrome was assessed. Results. Serious complications of anesthesia were not recorded. The indices of central hemodynamics remained stable during anesthesia, the indices of stress markers decreased somewhat, which indicates the adequacy of both variants of anesthesia. Twelve hours after the operation, a significant increase in the cardiac index, heart rate and blood pressure, as well as an increase in the concentration of cortisol and glucose, was noted in Group II patients who had anesthesia with bupivacaine. This was accompanied by an intensive pain syndrome, which required an additional appointment of tramadol. In group I patients (caustic anesthesia with ropivacaine), the indices of central hemodynamics and stress markers remained stable after the operation, and the intensity of the pain syndrome was significantly lower. Conclusions. Caudal anesthesia with 0.75% ropivacaine is a highly effective, reliable and safe method of anesthesia in traumatological operations on the lower extremities in children. Caudal administration of a 0.75% ropivacaine provides a longer sensory and motor block compared with a 0.5% bupivacaine. The superiority of caudal anesthesia with ropivacaine is due to the more potent and prolonged action of the drug, as evidenced by the wider spread of the sensory block, deeper motor blockade, stability of hemodynamic parameters and stress markers.</p></abstract><trans-abstract xml:lang="ru"><p>Цель исследования. сравнительное изучение каудальной анестезии бупивакаином и ропивакаином при оперативных вмешательствах на нижних конечностях у детей. Материал и методы. В исследование включено 59 детей, оперированных в условиях каудальной анестезии по поводу врожденного вывиха бедра, деформаций костей таза, косолапости и др. Больные были разделены на 2 группы в зависимости от вида применяемого местного анестетика: в 1-й группе (n=39) проводилась каудальная анестезия 0,75% раствором ропивакаина с в/в седацией пропофолом, во 2-й группе (n=20) каудальный блок проводился 0,5% раствором бупивакаина. Во время операции и в послеоперационном периоде исследовались показатели центральной гемодинамики, концентрации кортизола и глюкозы плазмы крови, оценивалась интенсивность послеоперационного болевого синдрома. Результаты. Серьезных осложнений анестезии зафиксировано не было. Показатели центральной гемодинамики во время анестезии оставались стабильными, показатели маркеров стресса несколько снижались, что свидетельствует об адекватности обоих вариантов анестезии. Спустя 12 ч после операции у пациентов 2-й группы, которым анестезия проводилась бупивакаином, отмечено значимое повышение сердечного индекса, частоты сердечных сокращений и артериального давления, а также увеличение концентрации кортизола и глюкозы. Это сопровождалось интенсивным болевым синдромом, требовавшим дополнительного назначения трамадола. У пациентов 1-й группы (каудальная анестезия ропивакаином) показатели центральной гемодинамики и маркеров стресса оставались стабильными и после операции, а интенсивность болевого синдрома была значимо ниже. Заключение. Каудальная анестезия 0,75% раствором ропивакаина - высокоэффективный, надежный и безопасный метод обезболивания при травматологических операциях на нижних конечностях у детей. Каудальное введение 0,75% раствора ропивакаина обеспечивает более длительный сенсорный и моторный блок по сравнению с 0,5% раствором бупивакаина. Преимущество каудальной анестезии ропивакаином обусловлено более мощным и длительным действием препарата, о чем свидетельствуют более широкое распространение сенсорного блока, более глубокая моторная блокада, стабильность показателей гемодинамики и маркеров стресса.</p></trans-abstract><kwd-group xml:lang="en"><kwd>caudal block</kwd><kwd>ropivacaine</kwd><kwd>bupivacaine</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>каудальный блок</kwd><kwd>бупивакаин</kwd><kwd>ропивакаин</kwd></kwd-group></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Сичкарь С.Ю.
