Modern approaches to preoperative cardiovascular risk assessment, while accurately predicting overall risk level, doesn't help anaesthesiologist to choose anaesthesia methods in exact patient. The authors analyzing novel approach, based on non-invasive monitoring assessment of hemodynamic response to the stimuli, physiologically similar to those challenging patient's condition during the surgery and anaesthesia. In addition to the previously introduced prognostic model of hemodynamic response to central neuraxial block based on nitroglycerine test, esmolol test as a model of response to myocardiodepressive action of general anaesthesia induction is proposed.</p></abstract><trans-abstract xml:lang="ru"><p>В обзоре обсуждается проблема прогнозирования реакций системной гемодинамики на индукцию общей анестезии и центральные нейроаксиальные блоки. Существующие методы дооперационных функциональных исследований кровообращения, вполне удовлетворительно оценивающие уровень риска сердечно-сосудистых осложнений в целом, мало помогают анестезиологу в принятии решений по выбору тактики у конкретных пациентов. В качестве решения проблемы авторы анализируют подход, основанный на неинвазивной мониторной оценке реакций системной гемодинамики на стимулы, физиологически подобные тем возмущающим воздействиям, которые имеют место далее во время операции и анестезии.
