This needs a multidisciplinary approach: surgeons reduce the invasiveness of surgery, and anesthesiologists use a multimodal approach in the intraoperative and postoperative periods. They combine general anesthesia with epidural anesthesia (EA), which has not only undeniable and proven advantages but also several disadvantages. An interesting alternative is paravertebral block (PVB), a compromise between neuroaxial (EA) and peripheral nerve block. PVB provides analgesic efficacy comparable to that of EA, but with significantly fewer complications and side effects. Many studies have described the widespread and successful use of PVB in thoracic surgery and endoscopic urology. However, the literature on the use of PVB in laparoscopic renal surgery anesthesia is extremely scarce. Thus, large multicenter studies are needed to determine the place of PVB in the anesthetic management of laparoscopic renal surgery.</p></abstract><trans-abstract xml:lang="ru"><p>Одна из основных целей современной анестезиологии – снижение стрессовой реакции организма на хирургическую агрессию. Реализация её возможна за счёт мультидисциплинарного подхода. С хирургической стороны это достигается уменьшением инвазивности операции, с анестезиологической – за счёт мультимодального подхода как в интра-, так и в послеоперационном периодах. Достаточно распространённой тактикой является сочетание ОА с эпидуральной анестезией (ЭА), которая, однако, при всех своих неоспоримых и доказанных преимуществах имеет ряд недостатков. В качестве достойной альтернативы особый интерес представляет паравертебральная блокада (ПВБ), которая является своеобразным компромиссом между нейроаксиальной и периферической нервной блокадой. ПВБ характеризуется сопоставимой с ЭА эффективностью анальгезии, но существенно меньшим количеством осложнений и побочных эффектов. ПВБ широко и с успехом применяется в торакальной хирургии, эндоскопической урологии, о чём свидетельствуют данные многочисленных исследований. В то же время литература, посвящённая использованию ПВБ в качестве компонента анестезии при лапароскопических операциях на почках, крайне немногочисленна.
