However, one of the elements of the technique is the use of an intraarticular or wound catheter to prolong the analgesic effect.</p> <p><bold><italic>AIM</italic></bold>: Therefore, the investigation aimed to estimate wound catheter use as a component of LHVIA in TKA.</p> <p><bold><italic>MATERIALS AND METHODS</italic></bold>: The study included 102 patients who underwent primary TKA for stage 3 gonarthrosis under spinal anesthesia with intravenous sedation. Two groups were formed by envelope randomization: the LHVIAc group (n=51) in which the wound catheter was placed at the end of the operation and followed by the infusion of the local anesthetic solution for 24 h; and the LHVIA group (n=51) in which the patients were treated without a catheter.</p> <p><bold><italic>RESULTS</italic></bold>: The groups were comparable in terms of sex, age, and the nature of the pathology. The LHVIAc group showed a significantly decreased severity of pain in 4–6 h after surgery, both at rest and during flexion of the knee (p=0.001 and p=0.003). On postoperative day 1, the use of the wound catheter contributed to a significantly decreased severity of pain syndrome when flexing the knee (p=0.002). No infectious complications or significant adverse reactions were recorded in the groups.</p> <p><bold><italic>CONCLUSION</italic></bold>: 1) LHVIA is an effective method of postoperative analgesia in TKA; 2) the use of the catheter in the early postoperative period increased the analgesic effect of LHVIA without increasing the frequency of infectious and other complications; 3) the use of the wound catheter reduced the severity of pain syndrome in postoperative day 1 during activation, which facilitates postoperative rehabilitation of patients with TKA.</p></abstract><trans-abstract xml:lang="ru"><p><italic><bold>Обоснование</bold></italic>. Для послеоперационного обезболивания при тотальном эндопротезировании коленного сустава (ТЭКС) всё более популярным становится использование местной высокообъёмной инфильтрационной анестезии (МВИА). Одним из элементов методики является использование внутрисуставного или раневого катетера с целью пролонгации анальгетического эффекта.</p> <p><italic><bold>Цель</bold></italic>. Оценка целесообразности применения раневого катетера как компонента МВИА при ТЭКС.</p> <p><italic><bold>Материал и методы</bold></italic>. В исследование включены 102 пациента, которым было выполнено первичное ТЭКС по поводу гонартроза 3-й стадии в условиях субарахноидальной анестезии с внутривенной седацией. Путем рандомизации по методу конвертов были сформированы 2 группы: группа МВИАк (n=51), пациентам которой в конце операции инфильтрационная анестезия дополнялась установкой раневого катетера с последующей инфузией раствора анестетика в течение 24 ч, и группа МВИА (n=51), в которой катетер не применялся.</p> <p><italic><bold>Результаты</bold></italic>. Группы были сопоставимы по полу, возрасту, характеру основной и сопутствующей патологии. В группе МВИАк получено достоверное снижение выраженности болевого синдрома через 4–6 ч после операции как в покое, так и при сгибании коленного сустава (р=0,001 и р=0,003 соответственно). В 1-е послеоперационные сут применение раневого катетера способствовало достоверному снижению выраженности болевого синдрома при сгибании коленного сустава (р=0,002). Инфекционных осложнений и значимых побочных реакций в группах зафиксировано не было.</p> <p><italic><bold>Выводы</bold></italic>: 1) МВИА является эффективным способом послеоперационного обезболивания при ТЭКС; 2) применение катетера в раннем послеоперационном периоде позволяет усилить анальгетический эффект МВИА без увеличения частоты инфекционных и иных осложнений; 3) применение раневого катетера способствует снижению выраженности болевого синдрома в 1-е послеоперационные сут при активизации, что может способствовать проведению послеоперационной реабилитации больных при ТЭКС.</p></trans-abstract><kwd-group xml:lang="en"><kwd>local infiltration anesthesia</kwd><kwd>total knee arthroplasty</kwd><kwd>wound catheter</kwd><kwd>postoperative analgesia</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>местная инфильтрационная анестезия</kwd><kwd>тотальное эндопротезирование коленного сустава</kwd><kwd>раневой катетер</kwd><kwd>послеоперационное обезболивание</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Ovechkin AM, Politov ME, Panov NV.
